Corrections and updates are logged here, dated, and never silently overwritten. Published analysis pages also carry their own version and status stamps.

## 2026-09-28 — WISeR nine-month update and editorial revision

- New: **WISeR at Nine Months: What the Records Show** (wiser-nine-months.html), an update based on the CMS records released through FOIA litigation in September 2026, including a correction to the widely reported denial count (5,944 denials across two vendors in the first three months, not 20,000). The six-month WISeR analysis now links to it.
- The nine-month WISeR update is now the featured analysis on the homepage (hero link and analysis card) and is linked at the top of the six-month analysis page.
- Six-month WISeR analysis: model name corrected to "Wasteful and Inappropriate Service Reduction," matching CMS usage.
- Editorial revision across the site: plainer phrasing, fewer dashes, and assessments stated as the author's where they are opinions. No findings, figures, or confidence ratings changed, with one exception: on the Independent Governance page, "algorithm-driven coding practices" is now "Medicare Advantage risk-adjustment coding practices," which more accurately describes the settlements referenced.
- Rural health page: "trained in health law" corrected to "studying health law."
- GDAC Watch: the June 1, 2026 entry (CMS community engagement interim final rule, CMS-2454-IFC) was listed above the July entries in the tracking log; it is now in date order, with its Federal Register citation (91 Fed. Reg. 33348, June 3, 2026). The January 1, 2027 program change in the forward calendar was also moved ahead of the February 2027 report dates. No content changed.

## 2026-09-27 — Plain-language release

- New plain-language glossary (glossary.html). The first use of each acronym on every page now links to its definition, with the full name shown on hover.
- Homepage shortened by roughly two-thirds; a personal introduction with a photo now leads, and the About section is rewritten.
- New Services page holds the full service descriptions; published prices now show starting ("from") prices only.
- RUAIH Readiness Scorecard page now describes what the assessment covers and delivers, without publishing its item-level scoring method.
- The Rural-AI Deployment Evaluator is now the **Rural AI Readiness Check** (v2.0): written for rural health leaders rather than technical staff, with seven plain questions, a readiness result, a to-do list, and step-by-step instructions for saving results (copy and paste into any document, or print to PDF).

## 2026-09-26 — Site review release

**Credentials and accuracy**
- Removed AOTA and NCOTA references from For Clinicians (disclaimer and the "NCOTA 2026 session" call to action).
- Corrected the RUAIH Scorecard page: the practice history is in Illinois and Tennessee/Appalachia, not western North Carolina.
- Replaced "prescriber" with "evaluating clinician" (homepage About).
- Standardized the credential line sitewide: AIGP · OTR (NBCOT) · RESNA ATP/SMS.
- Added a licensure-status line to every page footer: NC occupational therapy licensure application pending; 5Q Health does not provide OT evaluation or treatment services.
- contact@5qhealth.ai is now the public address everywhere. 5qhealth@pm.me is stated on For Counsel and in the privacy notice as the channel for material that needs a higher level of security or encryption.

**Tools**
- Coverage Keeper v2.0: the January 1, 2027 Medicaid work and community engagement rule (80 hours or $580 a month; 3 consecutive months at application; 3 of 6 at renewal; 6-month renewals; 30-day notice; appeals), the full exemption list with proof to keep, a medically frail box, a printable monthly hour log, the retroactive-coverage change, updated SNAP work rules, and added free-help resources. Citations updated to P.L. 119-21, CMS-2454-IFC, S.L. 2026-1, and S.L. 2026-41.
- GDAC Watch launched: all section numbers and dollar figures verified against chaptered S.L. 2026-41; referenced case ledger added.
- 2029 Convergence Calculator: statutory basis verified against the chaptered law; the rate-floor description corrected to "100% of the lesser of the supplier's usual and customary rate or the maximum allowable Medicaid fee-for-service rate."
- Rural-AI Deployment Evaluator: site navigation, model card, version, and a case-status note added.

**Pages**
- Terms of use now state how content is made: AI tools assist research and drafting; all published work is verified and approved by Tabatha James.
- New: GDAC Watch, the NC Medicaid work requirement analysis (v1.1, adds the CMS interim final rule), RHTP Governance Partnership, Reading Room, Privacy, Terms, Accessibility.
- Homepage repositioned around occupational justice, person-centered clinical judgment, and AI/data governance for rural and safety-net providers, working alongside counsel. The Fifth Question is now on the homepage.
- For Counsel: now covers in-house, outside, and retained counsel, and adds a regulatory and compliance engagement type performed at counsel's direction.
- WISeR analysis: status note for the July 16, 2026 Senate vote (motion to proceed to S.J.Res.198 rejected 46–50).
- Independent Governance: NC H565 status as of September 26, 2026.
- HTI-5 analysis: rule status re-verified September 26, 2026 (still proposed).
- Site navigation and footer made consistent on every page; favicon added; sitemap updated.
